Faculty who want to find out what challenges students are facing can utilize a new web form created in Office365.\u00a0<\/span><\/p>\n<p><span style=\"color: #000000\">The form can be modified by faculty prior to sending out. The survey should take students 5 minutes to complete, and asks for the following types of information:<\/span><\/p>\n<ul>\n<li><span style=\"color: #000000\">whether students expect to have reliable Internet access<\/span><\/li>\n<li><span style=\"color: #000000\">times of day students expect to do online work<\/span><\/li>\n<li><span style=\"color: #000000\">preferences for asynchronous or synchronous activity<\/span><\/li>\n<li><span style=\"color: #000000\">accessibility requests (content in different formats, for example)<\/span><\/li>\n<li><span style=\"color: #000000\">basic psychological and physiological needs<\/span><\/li>\n<\/ul>\n<p><span style=\"color: #000000\">The survey form is available below.
